We are happy you have decided to bank your child’s cord blood with our company Cord Blood Center. Below you can find detailed Step by Step guide for successful cord blood collection, processing and storage.
Cord Blood Banking Step by Step
Step 1: Arrangement with your Maternity Hospital
You should inform your obstetrician or other responsible medical staff in maternity hospital that you are interested in your child’s cord blood collection. If cord blood collection has to be approved by the hospital management you should obtain it.
Inform your obstetrician or medical staff that you will bring your own cord blood collection kit.
We will send you all necessary documentation in your chosen language (EN, DE, FR). Prior to cord blood collection you should pass this documentation to responsible medical staff and ask them to fill it accurately.
The documentation includes:
- Cord Blood Collection Instruction;
- Cord Blood Collection Slip;
- Application for Cord Blood Collection (for hospital).
Step 2: Enrollment for Cord Blood Collection
After you have arranged approval for cord blood collecti on from hospital you should enroll with our company Cord Blood Center. Enrolment is necessary step to make a contract
between you and our company to smoothly carry out with cord blood collection, processing and storage for your child.
You can enroll by:
a) Enrollment Form (English)
b) E-mail: frantova@cordcenter.com
Please put down into Enrollment Form Your comments and questions or into your e-mail message information, which language you have chosen for all documentation that you will receive. It´s possible to choosen: English, German, France or Slovak language version.
Your comments and questions in Enrollment Form or in your e-mail message please write only in
- English or
- German or
- Hungarian or
- Slovak.
Step 3: Cord Blood Collection Kit and documentation
You have two options how to obtain the Collection Kit. Please write your decision to frantova@cordcenter.com or into Your comments and questions section of the Enrollment Form.
a) You can pick up your Collection Kit personally on Mokrohajska 6, 831 53, Bratislava, Slovakia. This can be done by you or by your representative carrying ID card. Please inform us at least 1 day ahead about his/her name. Personal collection is free of charge.
b) The Collection Kit will be sent to you by currier to the address listed in your Enrollment Form (Your comments and question section) or by email. It will be dispatched in 3 working days after receiving your request. The cost of shipment is determined by your address.
Together with the Collection Kit you will receive documentation:
- Cord Blood Collection and Storage Contract
- Questionnaire
- Transport Agreement
Step 4: Information about Cord Blood Collection
After cord blood collection is completed please inform us as soon as possible between 8.00 and 20.00 on +421 905 373 942 begin_of_the_skype_highlighting +421 905 373 942 end_of_the_skype_highlighting. You should report mother’s name and place of delivery.
Step 5: Cord Blood Transportation
After receiving information about successful cord blood collection our company Cord Blood Center will ensure the transport to our processing laboratory. The conditions and mode of transport will be declared in the Transport Agreement.
Step 6: Cord Blood Processing and Evaluation
It takes about 4 weeks to examine and test processed cord blood. Afterwards we will send you
- Cord Blood Processing Result
- Cord Blood Collection Invoice
- Cord Blood Collection and Storage Contract signed by our part.
Step 7: Payment
If processed cord blood is suitable for transplantation and you have decided for storage you should pay basic fees according to Cord Blood Collection and Storage Contract based on send Invoice. It is possible to pay in single payment or 4 month installments.
Additionally you will be charged annual storage fee according to Cord Blood Collection and Storage Contract based on send Invoice.
